Hospice care can be provided to residents in skilled nursing facilities (SNFs). Our Hospice-SNF Coordination of Care program is a collaborative partnership between our hospice teams and the nursing home teams to provide opportunities for seamless coordination of the highest quality care.
Hospice staff at nursing homes receive special training to effectively manage the discomfort and distress felt by hospice residents, and they will work with your doctor to make sure your loved one is not in pain.
Some benefits of hospice services provided in a nursing home are:
• Hospice staff are specially trained to anticipate, assess and treat all types of physical symptoms that cause discomfort and distress.
• Hospice staff can collaborate with nursing home staff on a palliative plan of care.
• The resident has access to professionals in all disciplines who can help manage pain and symptoms.
• Hospice staff augment the care provided by nursing home staff and assist with activities of daily living.
• Nursing facility staff are often seen as an extension of the resident’s family and friends. While the primary focus is always on the hospice patient and his or her family and friends, hospice staff also provide emotional support and training in end-of-life care issues to their colleagues in the nursing home.
• With the support of hospice, the nursing home can care for the resident until the end of life, preserving dignity and ensuring comfort.
• Some costs related to the terminal illness are covered hospice benefits under many healthcare programs.
